About the Role:

The Sr. Associate, Nursing & Social Care will support the nursing and social care organization and be responsible for the development of tools and assets that support nursing and social care, project manage and organize teams and serve as a lead in communicating critical updates across teams.

Essential job responsibilities:

  • Drive coordination and execution of individual projects through to completion and centering on the foundational elements of the ideal Cityblock Nursing & CHP Experience
  • Lead the content development and asset creation to support the implementation of a monthly “core principle” to drive ongoing effectiveness of our nursing roles
  • Maintain tools and resources including Common Ground Landing page, Monthly Newsletter and Cross-organizational updates
  • Organize content and material for Nursing CoP and CHP CoP
  • Partner with Social Care Lead and team to translate foundational elements into CHP role development in a consistent, standardized manner
  • Support the management of nursing performance goals, tracking progress across the team, uncovering and escalating performance gaps, and partnering with clinical SMEs to fine tune workflows in order to achieve target effectiveness
  • Design a pilot framework for market-based local initiatives based to address member needs and drive impact, including a clear measurement set, test & learn process, clear timeframes, and decision process to scale learnings
  • Manage communications and progress updates to leadership, collaborators, and front-line staff through business memos, briefs, decks and/or other methods through engaging, well-organized presentations
  • Drive connectedness to relevant collaborating teams to support performance management needs and enable execution at scale across key initiatives

Requirements for the Role:

  • Bachelor’s required
  • Healthcare background highly preferred
  • Minimum of 4-5 years of project, process and/or operations management
  • Skilled in using Google Suite and Mac OS
  • Skilled in analyzing and synthesizing data
  • Understanding of how to use electronic health record systems and/or care facilitation platforms
  • Proficient in analyzing member clinical and demographic data
  • Strong problem solving skills - can make difficult decisions and knows when to collaborate with other team members
  • Able to provide creative solutions to challenges within the healthcare system that are impeding optimization of members’ care and health
  • Growth and learning mentality, ability to think outside the box, go outside the bounds of “traditional” responsibilities
  • Adaptable to change and prepared for frequent, fast-paced changes and shifting priorities

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The expected salary range for this position is $72,891 to $91,000. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
