About the Role:
The Mobile Integrated Care team radically improves healthcare for individuals by providing access to compassionate providers, without barriers, in the community where the member lives. With the goal of delivering the right care, at the right time, and in the right place. Often avoiding hospitalizations or connecting the member to longitudinal care. The Manager, Community Care will play a vital role in delivering this model of care by making sure the clinicians seeing members are well equipped, operationally ready, certified, trained, and effective..
Responsibilities
Maintain a cost effective operation that drives down total cost of care through execution of all MIC service lines within the market
Track and report key performance metrics. Identify performance gaps and create solutions to meet performance goals.
Liaise with market leaders and foster a collaborative working relationship with stakeholders
Maintain a full workforce of motivated and effective team members

Work closely with VUC to identify:
Service offerings, limitations, opportunities, volume, clinical issues

Conduct regular meetings and present operational and clinical updates to:
Market leaders
MIC leadership & partners
Field Staff

Hold regular 1:1s with all field staff
Assign goals, plan strategically for employee success
Identify improvement opportunities
Highlight achievements and identify growth opportunities

Review field personnel KPI’s
Licensing/certification, Attendance, Documentation, Equipment, clinical feedback
Optimize daily schedules to reduce travel, redundancy, mitigate conflicts, call outs, late additions and special requests.
Perform payroll, keep close watch of employee hours, overtime, PTO use

CQI/QC Activities

Review Athena and Commons notes for accuracy and compliance:
Written consent
ED@Home clinical workflow (NEWS/911 Activation)
Documentation of written notes, completion of orders, assignment of tasks, billing codes and note closure.
Flag cases for clinical review by the MIC medical director or their designee

Manage a schedule:
Employee assignments, geography, workforce distribution, workforce interruptions, staffing models, future planning.

Onboard new employees:
Train on clinical competencies
Track Docebo learning
Create 4 week field training plan with preceptors
Equipment and vehicle assignments

Create and manage inventory tracking systems including all supplies kept in the office and out in the field.
Track and manager employee inventory holding accountability for equipment and medication management
Track preventative equipment maintenance and establish accounts with vendors to service equipment

Work with Human Resources/ SBP to:
Create employee performance improvement plans
Off board employees

Hire new employees
Getting approval for all new FTE positions
Post, interview and hire candidates aligned with Cityblock values

Outreach:
Strategize with Outreach Services to utilize MIC field teams in the outreach process
Work with the TOC team to reach hard to reach members using MIC outreach
Work with care teams within the market to outreach high utilizers, lost to care, disengage, and high risk/stratified members

Vehicle Management
Manage a fleet of vehicles keeping them in operationally ready
Serviced, equipped, and well maintained
Account for miles, fuel expenditure, use and signs of misuse or abuse

Project management
Manage market and central operations projects either self initiated or as assigned.

Track progress, align with stakeholders, create transparent work progress, deliver within stated timelines. Monitor effectiveness.
Training other managers, market partners, field employees, and leadership

Clinical Responsibilities: The CCM, Manager of Community Care is the principal clinical authority for field operations in the market and must be well versed in all clinical offerings and limitations. Including medications, treatments, scopes of practice, relevant local laws and protocols, and treatment modalities. As well as be a clinical innovator, identifying gaps that could be closed with new services, medications, treatments or workflows.
Perform 3 clinical shifts per work month (20 days) to equal 15% clinical time

Is proficient to the level of being able to teach the following:
IV Placement, infusions, PICC line access
General phlebotomy in hard to access and pediatric populations
Medication Administration - Indications, contraindications, common doses and routes, side effects, reversal agents
Point of care tests
COVID/FLU, Strep, Urine DIp/TOX, HCG, Wound culturing, Glucose
Physical examinations
Neuro exams
LAI administration
Treatment for all local protocolized diseases such as CHF, COPD, Sepsis, arrhythmia, allergic reactions, etc..
Can read and interpret EKG’s and rhythm strips including identifying STEMI
Identify all major structures in an ENT exam
Skilled at using a video otoscope
Gives clear and concise verbal reports in a SBAR or similar format
Is an expert at using the NEWS score in an integrated model of care following Cityblocks ED@home clinical workflow.
Experience in non-traditional healthcare roles
Remove sutures
Irrigate ear canal after otoscopic exam
Troubleshoot foley catheters
Perform CPR as a BLS provider
Operate a motor vehicle in a safe manner
Read a map and use GPS
Experienced with talking to members about difficult medical topics such as end of life care, diseases, behavioral health, substance use, and social determinants of health.

Proficient in the Google suite of products including Docs, sheets, slides
Proficient in MS Office products, Word, Excel, Powerpoint

Able to understand the relationship between data and operations, running reports on all market activities and how individual, group, and collaborative efforts affect outcome.

Is an excellent resource manager with experience in workforce utilization, planning growth markets, building infrastructure remotely, training a workforce remotely.
Has excellent analytical abilities that can be applied to real life applications

Can learn new software and hardware applications in a rapidly changing environment

Has experience managing a diverse workforce of healthcare clinicians

Experience with emphasis on non-traditional and geographically dispersed workforce, managing via remote.
Is experienced working with all levels of healthcare clinicians including physicians, behavioral health professionals, nurses, PA’s, NP’s, MA, EMTs, Paramedics and LPNs.

Education:
Minimum Education: High School Diploma, some college
Ideal Education: Bachelor's degree or higher

Paramedic certification in state where operating clinically
BLS CPR

Good to have: ACLS, PALS, AMLS, PHTLS, GEMS
Certified Community Paramedic (CP-C) with the IBSC within 1 year
Valid state driver's license without restrictions

Required to travel as needed for the role. Including off-site department meetings and within market to member homes, hubs,

Climb stairs with 40 lb pack
Walk long distances from parking spot to visit address
Kneel, stoop, lean over for extended periods of time
Sit and ride in a vehicle for extended periods of time
Donning and doffing of PPE
Wear a N95 for extended periods of time

Able to see, hear, and smell to perform physical exams
