Sr. Associate, Network Integration

The Sr. Associate, Network Integration will be responsible for network integration activities including identifying, outreaching, and building relationships, and long-term relationship management on behalf of Strive Care Partners. This person will onboard and collaborate with both Nephrologist partners as well as community providers including specialty provider practices and facilities that contribute clinical value to our care model. The Sr. Associate will support the development and execution of the Network Integration centralized playbook for how to effectively work with these providers across SCP (Strive Care Partners) SC365 and CKCC markets in coordination with the Market Operations leads. Sr. Associate, Network Integration will play a crucial role in driving seamless collaboration and optimizing Nephrology Provider and Specialty Provider engagement initiatives that contribute to the overall growth and success of SCP.

Essential Functions

Nephrologist Provider Relations

  • Work with internal Strive providers to collaborate and build relationships with Nephrologist partners to establish and active referral pipelines
  • Facilitate relationship building among SCP providers and Nephrologists to promote collaboration, knowledge sharing, and best practices of SCP
  • Develop and implement tracking mechanisms to monitor the success and impact of provider engagement initiatives in each market
  • Analyze data, generate reports, and identify trends and insights and use findings to propose actionable recommendations and refine strategies for continuous improvements
  • Work with market operations to develop standardized reporting and communication to key stakeholders in market
  • Support initiatives regarding provider facing roadshow sessions to help educate providers on SCP care model
  • Work closely with internal teams, including sales, marketing, operations, and technology, to align strategies and initiatives
  • Collaborate on joint projects and initiatives to maximize integration efforts and enhance overall organizational performance
  • Meet and exceed targets for physician engagement activities

Specialty Provider Relations

  • Develop and execute strategies to effectively engage healthcare providers including SNFs, hospitals, rehab facilities, cardiologists, endocrinologists, and other types of community providers
  • Collaborate with internal teams to identify provider needs, address concerns, and ensure satisfaction
  • Conduct educational sessions during outreach around the benefits of Strive Care Model
  • Collaborate and build relationships with local market providers to establish and activate referral pipelines
  • Identify potential partnerships and implement streamlined processes for seamless patient referrals
  • Foster long-term relationships by providing support in promoting value in SCP model
  • Coordinate the integration of EMR systems with external clients, including healthcare providers and facilities
  • Stay updated on industry trends, market developments and activities, to inform strategy and stay ahead in market
  • Meet and exceed targets for physician engagement activities

Minimum Qualifications

  • 3+ years of healthcare experience, with specific experience in engaging with Providers
  • Bachelor’s degree
  • Ability to travel locally up to 75% of the time

Preferred Qualifications

  • Master’s degree
  • 3+ years management experience
  • Ability to be strategic as well as tactical
  • Excellent written, oral, and presentation skills
  • Entrepreneurial spirit and comfort in ambiguity
  • Proven to be a self-starter
  • Comfortable presenting to Providers and driving the conversation, answering unscripted questions, and building relationships based on subject matter expertise
  • Comfortable reviewing data and reporting with provider groups and using analytics to drive performance improvement
  • Able to identify a problem, develop, plan, and execute to resolution, and communicate progress to stakeholders
  • Comfortable with project management techniques and able to develop a project plan that includes collaborating with market leadership and supporting departments
  • Highly responsive to Providers as “customer” being a single point of contact for valuable relationships
  • Persistence getting to “yes” in the face of many “no’s”
  • Comfortable working as a member of team in high paced startup environment
  • Outgoing, sales focused skill set

Annual Salary Range: $72,800.00-$88,300.00
